Registration Forms for Transfer Students

Step 1 

  • Complete the registration form below. Three emergency numbers are needed, excluding parent/guardian, who are available to pick up student if ill or injured.  Parents are always contacted first.

Please be prepared to attach:

  • Immunization Record – Attach a copy of your child’s immunization records from your doctor and/or school nurse.
  • Birth Certificate – Attach a copy of your child’s city/state issued birth certificate (no hospital-issued birth certificates will be accepted).
  • Supporting Custodial and/or Guardianship documents if applicable.

  • Registration Fee – There is a one-time non-refundable fee that must be paid at the time of registration in the amount of $750:
